Dialysis Australia’s home dialysis model of care – increased choice and convenience in where people receive their care

The innovative model of Assisted Home Haemodialysis (AHHD) brings the hospital to the home of people living with Chronic Kidney Disease (CKD), providing greater flexibility regarding treatment times and eliminating the need to travel to and from hospital. The breakfast session will highlight key elements of the program, its future direction, referral pathways, clinical governance and collaboration with key stakeholders. Dialysis nurses, nurse managers and Nephrologists are encouraged to attend. Attendees will have a better understanding of the AHHD initiative leading to informed decision making for suitable patients and their families regarding their transition home.

The ASM is hosted by Australian and New Zealand Society of Nephrology.

The aims of the Society are to promote and support the study of the kidney and urinary tract in health and disease, and to ensure the highest professional standards for the practice of nephrology in Australia and New Zealand.
